Your forms process is not finished until the 2nd signer completes their portion of the forms. A link for the 2nd signer has been sent to the email address provided on the DocuSign start page.

Are you starting fertility treatment?

Cryo-Logix will keep your forms on file, and your storage account will be activated closer to the time when your specimens will be shipped for for storage, depending on your treatment plan.

Are your specimens already frozen?

Cryo-Logix will begin setting up your account and coordinating the shipment. Transfers take some time, but rest assured we are working on it. We will reach out to you if we have questions.

Payment

Just before shipment of your specimens, we will use the credit card you provided for payment of your first storage fee and any shipping or liability protection fees that you elected to purchase. If you need to make any changes to your payment method or contact information, please contact us.
